Dear 3K, Pre-K Families,
Each year, all NYC public schools ask families to complete the Family Income Inquiry Form (also known as the NYC School Meals Application).
Even if your child does not eat school breakfast or lunch, filling out this form is very important. The information you provide helps our school receive critical funding for programs, staffing, classroom resources, technology, and other services that benefit every student. Completing the form also helps determine your school’s Title I status and ensures that your school continues to receive essential support.

Only one form is needed per household, and all information is kept strictly confidential.
